Dear Dr. [name redacted],
You treated me for myxoid liposarcomas in 1993 and 2001. I will always be grateful for the kindness and expertise you extended to me during those episodes.
Two weeks ago an echocardiogram was performed that a local cardiologist indicates reveals a large tumor attached to the mitral valve in my left ventricle. Unfortunately, at least two errors were made in his initial report that have cast a cloud of uncertainty over its overall accuracy. Naturally, because of my high regard for you, I would like to confer with you at the earliest opportunity regarding the following:
1. Confirmation of diagnosis;
2. Treatment approaches;
3. Additional diagnostic testing that should be performed;
4. Prognosis.
Toward this end, I have forwarded a disk of the ultrasound to your office, sent three prior faxes, and phoned numerous times. I fully appreciate that you are an extremely busy man, but the apparent urgency of the situation has resulted in significant pressure on me from family to move forward in a constructive manner. Accordingly, I would very much appreciate a substantive response from your office before the end of this week.
Thank you so much for your continued courtesy.
Very truly yours,
PG
